Covered Services Terminology
A quick-reference glossary for interpreting conversations about healthcare services, support benefits, and coverage conditions.
Covered services are services or items a program or plan may pay for when applicable conditions are met. The categories below describe terminology heard during benefit inquiries; they do not mean every category is included in every plan.
Medical and preventive services
- Primary care services: Routine assessment, treatment, prevention, and coordination provided by a primary care professional.
- Specialist services: Care focused on a particular condition, body system, or field of medicine.

- Outpatient services: Care provided without formal inpatient admission.
- Inpatient hospital services: Hospital care provided following formal inpatient admission.

- Durable medical equipment (DME): Reusable equipment used for a medical purpose under the relevant program’s definition.
- Medical supplies: Items used in treatment or personal medical care, often consumed or replaced with use.
- Hospice services: A model of care supporting comfort and other needs near the end of life under the relevant program’s eligibility and election rules.
- Non-emergency medical transportation (NEMT): Transportation to or from medical services when there is no medical emergency, under applicable program requirements.
Other benefits and coverage conditions
- Dental services: Assessment and treatment related to teeth, gums, and oral health.
- Vision services: Services relating to eyesight and eye care, potentially including examinations or corrective lenses.
- Hearing services: Services relating to hearing assessment, treatment, or devices.
- Service limit: A restriction on the amount, frequency, or duration of a benefit.
- Site-of-service requirement: A rule affecting the setting in which care is covered or authorized.
- Separately administered benefit: A benefit arranged through another organization or coverage component rather than the member’s usual medical plan.
- Benefit verification: Checking whether a service is included and which conditions apply to the member’s coverage.
Service categories, eligibility, cost sharing, provider rules, and authorization requirements vary by plan and program. A plan representative can confirm coverage for the specific service and date. A service appearing in a glossary, provider recommendation, or general benefit category does not by itself establish coverage for an individual member.
