# Eligibility and Enrollment Terminology

## A quick-reference glossary for interpreting conversations about qualifying for coverage, joining a plan, and maintaining enrollment.

Eligibility means meeting a program’s requirements. Enrollment is the process of joining or being registered for coverage. An application, an eligibility approval, and a coverage start date are related but different events.

### Applying and qualifying

- Eligibility: Meeting the requirements for a program, benefit, or plan.
- Application: A request for coverage or benefits, with information needed to assess eligibility.
- Applicant: A person applying for coverage or benefits.
- Eligibility determination: A decision about whether a person meets program requirements.
- Verification: Checking information against documents or other approved sources.
- Supporting documentation: Records requested to establish facts relevant to an application.
- Household income: Income counted for the relevant household under a program’s rules.
- Resources or assets: Money or property considered in programs that use resource limits.
- Residency: Living in a location in a way that meets the applicable program’s requirements.
- Pending application: An application for which a final decision has not yet been completed.

### Enrollment and coverage dates

- Enrollment: Joining or being registered in a health program or plan.
- Initial Enrollment Period: The first defined period in which an eligible person may enroll under a program’s rules.
- Open enrollment: A recurring period during which specified enrollment or plan changes are permitted.
- Special Enrollment Period: An enrollment opportunity arising from a qualifying circumstance under the applicable rules.
- Effective date: The date coverage or a plan change begins.
- Service area: The geographic area in which a plan accepts eligible members and operates.
- Enrollment confirmation: A notice confirming registration in a plan or program.
- Member identification card: A card containing information used to identify a member and their coverage.

### Keeping or changing coverage

- Renewal: A process for continuing coverage into another period.
- Redetermination: A review of whether a person continues to meet eligibility requirements; in other contexts, the same word can name an appeal stage.
- Disenrollment: Ending membership in a plan.
- Termination of coverage: The end of coverage under specified rules or circumstances.
- Reinstatement: Restoration of coverage or membership under applicable conditions.
- Coverage gap: A period without applicable health coverage; in older Part D materials, the phrase may refer to a historical drug benefit phase instead.
- Change reporting: Informing a program of changes that may affect eligibility or enrollment.

Eligibility rules, enrollment opportunities, renewal requirements, and effective dates differ across Medicare, Medicaid, employer coverage, and other plans. The relevant agency or enrollment team can confirm individual status and deadlines. Medicaid applications are generally accepted throughout the year, but Medicaid managed care plan-change rules are separate.
